Information to be included in the declaration
Article 3
The declaration shall contain the following information: (a) the name and signature of the owner or of the person who is authorised in writing by the owner to carry out the non-commercial movement of the pet animal on behalf of the owner; (b) the place and date of issuance; (c) the unique alphanumeric code displayed by the transponder identifying each pet animal; (d) the number of the pet travel document accompanying each pet animal; (e) a commitment by the owner or the authorised person that the pet animal entering into Northern Ireland from other parts of the United Kingdom will not be subsequently moved from Northern Ireland to a Member State.
