Decision 94/741/EC shall be amended as set out in the Annex to this Decision.

Article 2

Decision 97/622/EC shall be amended as set out in the Annex to this Decision.

In the Annex to Decision 94/741/EC, the second questionnaire is amended as follows:

The title of questionnaire is replaced by the following:

‘QUESTIONNAIRE

for the report of the Member States on the transposition and implementation of Directive 2006/12/EC’;

2.

Section II point 4 of the questionnaire is replaced by the following:
