Article 1

Member States shall use the report form laid down in the Annex to this Decision  ( 3 ) when providing the information on the implementation of Directive 2012/18/EU in accordance with Article 21(2) of that Directive.

Article 2

Implementing Decision 2014/896/EU is repealed with effect from 31 December 2026. References to the repealed Decision shall be construed as reference to this Decision.

Article 3

This Decision shall apply from 1 January 2027.

Article 4

This Decision is addressed to the Member States.
