ANNEX II TECHNICAL FORMAT FOR REPORTING INFORMATION ON MEDIUM COMBUSTION PLANTS COVERED BY DIRECTIVE (EU) 2015/2193 For the purpose of reporting the information set out in this Annex to the Commission, any reference to ‘Plant category’ should be understood as a reference to the table ‘Part 1, Plant categories’ set out in Annex I to this Decision. PART 1 Metadata 1.1 Member State Identification of the Member State submitting the report 1.2 Organisation of the Member State for the implementation of Directive (EU) 2015/2193 Description of the competent authority(ies) responsible for the implementation of Directive (EU) 2015/2193 (national level, regional level, local level), distribution of responsibilities 1.3 Reporting year Calendar year to which the reporting refers  ( 1 ) PART 2 Qualitative and quantitative information 2.1 Permits or Registration Method, referred to in Article 5(1) of Directive (EU) 2015/2193, in use into the reporting year for each plant category (use of permit, registration, or both) 2.2 Procedure Procedure for granting a permit or for registration in respect of Article 5(3) of Directive (EU) 2015/2193, in use in the reporting year for each plant category (description of procedure, and URL to the procedure if available) 2.3 Register Presence of a register with information on each medium combustion plant pursuant to Article 5(5) of Directive (EU) 2015/2193 used into the reporting year for each plant category (YES or NO), if YES, Description of the register (number of registers, level of the registers – national, regional, local) Are the registers available to the public via the internet? (YES or NO) If yes, URL of register(s) 2.4 General binding rules Adoption of general binding rules in respect of Article 5(6) of Directive (EU) 2015/2193 and a reference to such rules ( description of use and URL available to the public ) 2.5 Number of plants part of small isolated systems (SIS) / micro isolated systems (MIS) Number of plants that are part of a small isolated systems (SIS), or micro isolated systems (MIS) as defined in Article 3 of Directive (EU) 2015/2193, into the reporting year for each plant category 2.6 Use of exemptions Exemptions applied in respect of: Article 6(3) of Directive (EU) 2015/2193 Article 6(8) For each exemption scheme, number of concerned plants. 2.7 Stricter emission limit values Overview of cases where stricter emission limit values are in force in respect of Article 6(9) of Directive (EU) 2015/2193 in the reporting year (brief description of reasons and procedures for application of stricter ELVs). This information is reported for each plant category when this information is available at this level. 2.8 Changes to medium combustion plants Examples of measures referred to in Article 9 of Directive (EU) 2015/2193 aimed at ensuring that the operator informs the competent authority of planned changes to the medium combustion plants which would affect the applicable emission limit values 2.9 System for compliance check Method used to verify that emissions monitored, with continuous or periodic measurements, do not exceed the emission limit values referred to Article 6 of Directive (EU) 2015/2193 (description of the system referred to in Article 8 of Directive (EU) 2015/2193 such as use of inspections, self-measurements, external measurements or other measures, use of accredited organisations to carry out controls). 2.10 Enforcement actions taken Overview of enforcement actions taken in the event of non-compliance as referred to in Article 8 of Directive (EU) 2015/2193, into the reporting year (brief description of the procedures established to enforce the Directive and types of enforcement actions), including the penalties set out in Article 16 of Directive (EU) 2015/2193. 2.11 Suspension of operations Summary of the type of cases of non-compliance referred to in Article 8 of Directive (EU) 2015/2193 having led to suspension of operations. ( 1 )   For the first report, preferably 2025. If not possible, 2024 or 2023, whichever year where the most recent data is available. For the second report, preferably 2030. If not possible, 2029 or 2028, whichever year where the most recent data is available.
