Veterinary Surgeons Act 1974 s 15
Applications for registration under this Act shall be in such manner and form and shall be accompanied by such documents, photographs, particulars and fees as the Council may prescribe, and proof of identity satisfactory to the Registrar shall also be furnished.
Permohonan untuk pendaftaran
Permohonan untuk didaftarkan di bawah Akta ini hendaklah dibuat mengikut apa-apa cara dan bentuk, dan hendaklah disertaikan dengan apa-apa dokumen, gambar foto, butir-butir dan bayaran sebagaimana yang ditetapkan oleh Majlis, dan bukti mengenai pengenalan yang memuaskan hati Pendaftar hendaklah juga diberikan.
