Counsellors Act 1998 s 32
(1) A registered counsellor whose certificate of registration or certificate of temporary registration, as the case may be, is lost shall immediately notify the Registrar of such loss. (2) The Registrar shall, on application being made and payment of the prescribed fee by the registered counsellor, issue to the registered counsellor a duplicate certificate of registration or certificate of temporary registration, as the case may be.
Perakuan pendua
(1) Seseorang kaunselor berdaftar yang hilang perakuan pendaftaran atau perakuan pendaftaran sementaranya, mengikut mana-mana yang berkenaan, hilang hendaklah dengan serta-merta memberitahu Pendaftar tentang kehilangan itu. (2) Pendaftar hendaklah, apabila permohonan dibuat dan fi yang ditetapkan dibayar oleh kaunselor berdaftar, mengeluarkan kepada kaunselor berdaftar itu suatu perakuan pendaftaran pendua atau perakuan pendaftaran sementara pendua, mengikut mana-mana yang berkenaan.
