(1) An application for a licence shall be made in writing to the Commissioner in such form as may be prescribed.
(2) Every application under subsection (1) shall be accompanied by such documents or information as may be prescribed and the Commissioner may, orally or in writing at any time after receiving the application and before it is determined, require the applicant to provide such additional documents or information as may be considered necessary by the Commissioner for the purposes of determining the suitability of the applicant for the licence.
(3) Where any additional document or information required under subsection (2) is not provided by the applicant within the time specified in the requirement or any extension of time granted by the Commissioner, the application shall be deemed to be withdrawn and shall not be further proceeded with, without prejudice to a fresh application being made by the applicant.

Permohonan bagi lesen
(1) Sesuatu permohonan bagi lesen hendaklah dibuat secara bertulis kepada Pesuruhjaya dalam borang yang ditetapkan.
(2) Tiap-tiap permohonan di bawah subseksyen (1) hendaklah disertakan dengan apa-apa dokumen atau maklumat sebagaimana yang ditetapkan dan Pesuruhjaya boleh, secara lisan atau bertulis pada bila-bila masa selepas menerima permohonan itu dan sebelum permohonan itu diputuskan, menghendaki pemohon supaya memberikan apa-apa dokumen atau maklumat tambahan
sebagaimana yang difikirkannya perlu bagi maksud menentukan kesesuaian pemohon bagi lesen itu.
(3) Jika apa-apa dokumen atau maklumat tambahan yang dikehendaki di bawah subseksyen (2) tidak diberikan oleh pemohon dalam masa yang dinyatakan dalam kehendak itu atau apa-apa pelanjutan masa yang diberikan oleh Pesuruhjaya, permohonan itu hendaklah disifatkan ditarik balik dan tidaklah boleh diteruskan lagi, tanpa menjejaskan pemohon untuk membuat permohonan baru.
