s 15 Application for registration
(1) A person who belongs to the class of data users as specified in the order made under subsection 14(1) shall submit an application for registration to the Commissioner in the manner and form as determined by the Commissioner.
(2) Every application for registration shall be accompanied with the prescribed registration fee and such documents as may be required by the Commissioner.
(3) The Commissioner may in writing at any time after receiving the application and before it is determined, require the applicant to provide such additional documents or information within the time as specified by the Commissioner.
(4) If the requirement under subsection (3) is not complied with, the application for registration shall be deemed to have been withdrawn by the applicant and shall not be further proceeded with by the Commissioner, but without prejudice to a fresh application being made by the applicant.

Permohonan untuk pendaftaran
(1) Seseorang yang berada dalam golongan pengguna data yang dinyatakan dalam perintah yang dibuat di bawah subseksyen 14(1) hendaklah mengemukakan suatu permohonan untuk pendaftaran kepada Pesuruhjaya mengikut cara dan dalam bentuk yang ditentukan oleh Pesuruhjaya.
(2) Tiap-tiap permohonan untuk pendaftaran hendaklah disertai dengan fi pendaftaran yang ditetapkan dan apa-apa dokumen sebagaimana yang dikehendaki oleh Pesuruhjaya.
(3) Pesuruhjaya boleh secara bertulis pada bila-bila masa selepas menerima permohonan dan sebelum permohonan itu diputuskan, menghendaki pemohon untuk memberikan apa-apa dokumen atau maklumat tambahan dalam tempoh masa yang dinyatakan oleh Pesuruhjaya.
(4) Jika kehendak di bawah subseksyen (3) tidak dipatuhi, permohonan untuk pendaftaran hendaklah disifatkan telah ditarik balik oleh pemohon dan tidak boleh diteruskan oleh Pesuruhjaya, tetapi tanpa menjejaskan suatu permohonan baru dibuat oleh pemohon.
