s 16 Application and issuance of licence
(1) An application for a licence shall be made to the Board in such manner as the Board may determine.
(2) The application under subsection (1) shall be accompanied by such documents or information as the Board may consider necessary and relevant to the application and the Board may, orally or in writing at any time after receiving the application and before the application is determined, require the applicant to provide such additional documents or information as the Board considers necessary for the purposes of determining the suitability of the applicant for the licence.
(3) The Board may, after receiving an application for a licence under subsection (1), issue the licence or refuse to issue the licence.
(4) Where an application for a licence is approved by the Board, the licence shall be issued in the name of the applicant upon payment of the prescribed fee and the Board may impose such terms and conditions as it may deems necessary.

Permohonan dan pengeluaran lesen
(1) Suatu permohonan bagi lesen hendaklah dibuat kepada Lembaga mengikut cara sebagaimana yang ditentukan oleh Lembaga.
(2) Permohonan di bawah subseksyen (1) hendaklah disertai dengan apa-apa dokumen atau maklumat yang difikirkan oleh Lembaga perlu dan berkaitan kepada permohonan itu dan Lembaga boleh, secara lisan atau bertulis pada bila-bila masa selepas menerima permohonan itu dan sebelum permohonan itu ditentukan, menghendaki pemohon itu supaya memberikan apa-apa dokumen atau maklumat tambahan yang difikirkan oleh Lembaga perlu bagi maksud menentukan kesesuaian pemohon bagi lesen itu.
(3) Lembaga boleh, apabila menerima suatu permohonan untuk lesen di bawah subseksyen (1), mengeluarkan lesen atau menolak untuk mengeluarkan lesen.
(4) Jika permohonan lesen itu diluluskan oleh Lembaga, lesen itu hendaklah dikeluarkan atas nama pemohon apabila dibayar fi yang ditetapkan dan Lembaga boleh mengenakan apa-apa terma dan syarat yang difikirkannya perlu.
