reg 9 Form of application

(1) An application for an order to be made under section 1 or 3 shall be made in writing and shall be addressed to the Secretary of State. (2) In the application the applicant shall give his name and the address to which all requests for further information, notices or other documents required to be served upon the applicant under the Act or these Rules are to be sent. (3) The application shall be signed by— (a) the applicant or, where the applicant is a body corporate, a director, the secretary or a duly authorised officer of that body; or (b) the authorised agent of the applicant. (4) In paragraph (3), “ director ”, in relation to a body corporate whose affairs are managed by its members, means a member of that body.
